Are you interested in joining the study and do not have an access code?
To get an access code, you must be a current Kaiser Permanente Northwest member. We need your name, date of birth, and medical record number because we will use a computer to collect information from your medical record (such as medical conditions, medications, height and weight) in order to determine if you might be eligible for the study. If you are eligible to take the screening survey, you will receive an email and/or letter with the unique access code that you can use to complete the web-based screening survey.
